The SF-1199A is a Direct Deposit Form that assists in facilitating the processing of your Direct Deposit Request. The Department requires a completed form and cover letter documented on letter head to be associated with each new or updated Direct Deposit, SF-1199A form. All entities doing business with the federal government are required to keep an active DUNS registration in the System for Award Management (SAM). An enhancement in G5 was implemented on January 28, 2016 in which program office users will be alerted that an applicant or grantee currently has an inactive status in SAM. Those entities with inactive DUNS numbers in SAM will automatically be placed on a Route Pay Flag requiring all payment drawdown requests be routed to the program office for approval until the DUNS registration is reestablished as "active".
All schools must complete the registration confirmation of their DUNS number annually.
In order to prevent delays in the receipt of Title IV program funds, we encourage all schools to complete their annual confirmation of their DUNS number with the SAM Web Site. The URL for the SAM Web Site is https://www.sam.gov.
